Every define now has a ;; comment explaining its purpose. Groups:

- Configuration and global state (57-67)
- Core evaluation functions (68-71)
- Special form helpers (72-89)
- CEK machine core (90-92)
- Special form CEK steps (93-119)
- Call dispatch and higher-order forms (120-133)
- Continuation dispatch (134-136)
- Entry points (137-140)
