gilesb ad15ef1ce7 Add explicit EFFECT node handling with case-insensitive lookup ...

- EFFECT nodes now handled explicitly like SOURCE, COMPOUND, SEQUENCE
- Case-insensitive node type matching throughout
- Fallback executor lookup tries both upper and original case
