acl: Phase 1 direct grants + deny-overrides, 24 tests

Datalog ACL layer (schema/facts/engine/api) over lib/datalog/. Direct
grant permits unless explicit deny names same (S,A,R) — deny-overrides
via stratified negation. Conformance wrapper + scoreboard.
