Browse Source 
- Remove default password fallback from POSTGRES_PASSWORD in docker-compose.yml
- Remove default password fallback from db.py and migrate.py
- Update .env.example with required POSTGRES_PASSWORD
- Update README to mark DATABASE_URL as required
